Program description: In partnership with Yakima County Probation Services, Comprehensive Healthcare will boost its ability to provide Behavioral Health and Substance Use Disorder (BHSUD) services. The Mental Health Supervision Team will direct individuals to BHS for assessments and to connect with their designated therapist. By placing BHS alongside the probation team, we aim to enhance communication and accessibility. First Responder Designated Crisis Responder Program: Comprehensive Healthcare shall coordinate with emergency responder and dispatching centers to establish Yakima County-wide designated crisis response teams. These teams will be tasked with providing an immediate crisis response as well as client-centered post-event care coordination. The primary objective is to reduce the need for First Responder responses to behavioral health calls, along with reducing their time spent on such calls. First Responder Designated Crisis Responder Program Teams' response availability is from 08:00am-11:00pm with flexibility as agreed by both parties. In coordination with all emergency/first responders and dispatching centers Comprehensive Healthcare will provide designated crisis response teams for individuals in a mental health crisis, substance use disorders, and/or co-occurring disorders.

Program description: Phase 1, the development phase, can commence upon funding. Involves a 1-year planning phase followed by a 1-year implementation phase. Planning integrates the new OT service line into TTS programming and establishes procedures for patient referral, documentation, billing, and data collection. If planning takes less than a year, implementation starts early. During implementation, patients receive OT services as part of interprofessional care. OT practitioners manage cases, supervise students, and collect evaluation data. By the end of the 2nd year, the program should demonstrate value to patients and become self-sustaining through customary billing practices. Program description: The program will provide Peer Recovery Support Specialist services to address the rising rates of substance use, drug and alcohol overdoses, and mental health challenges, in the community. Peer Recovery Support Specialist will deliver individualized, long-term wrap-around case management to at-risk populations, focusing on fostering trust and supporting recovery. These specialists will use their lived experiences to provide self-help, emotional support, and valuable information, building strong, trusting relationships with patients. Services will include goal setting, recovery planning, and walking alongside patients through mental/ behavioral health counseling, outpatient therapy, and substance use treatment to promote and sustain recovery. By enhancing Triumph's existing treatment options and strengthening connections to community resources, this program will improve outreach to homeless populations and increase treatment retention, recovery rates and overall service effectiveness in the Upper and Lower Yakima Valley. Program description: The District Court Probation Department will assign a Probation Case Manager to the Superior Court Mental Health Court Team. The Probation Case Manager collaborates with treatment providers to help participants achieve their Mental Health Court goals. They assist in implementing individual treatment plans and ensuring compliance with court-ordered terms. Responsibilities include monitoring release conditions, conducting urinalysis tests, tracking restitution payments, facilitating referrals to community services, and scheduling appointments. The Probation Case Manager attends all team meetings and court sessions. Participants meet with the Case Manager weekly in Phases 1 and 2, every other week in Phase 3, and as scheduled in Phase 4, although the schedule may vary based on individual needs as determined by the Mental Health Court Judge. Program description: The purpose of this contract is to provide educational and support sessions focused on Recognizing Trauma After an Overdose Fatality: Providing Support for Families. These sessions aim to address the emotional and psychological impact experienced by families and loved ones following an overdose fatality. By increasing awareness of trauma responses, the program will equip participants with tools to recognize and navigate grief, loss, and emotional distress in healthy and constructive ways. The sessions will emphasize building supportive environments, fostering resilience, and connecting families to resources for mental health, peer support, and counseling services. Through this initiative, the program seeks to promote healing, reducing stigma, and create a network of compassionate support for families navigating the aftermath of overdose-related trauma.
